This article addresses the critical issue of high caregiver turnover in home healthcare, citing that caregiver turnover rates reached an alarming 79% in Q4 2024. This turnover disrupts patient care continuity and increases risks of medication errors and hospital readmissions.
Key Recommendations for Case Managers:
- Transparency: Inform families about turnover realities upfront to manage expectations
- Family Education: Coach families to support caregivers through clear communication, proper feedback channels, and setting appropriate boundaries
- Critical First 72 Hours: Emphasize that success often depends on the initial three days, recommending families discuss expectations regarding dress code, pet care, phone usage, and housekeeping before services begin
- Communication Strategy: Help families identify agency contacts (supervisor, scheduler, nurse manager) for proper issue escalation
The author stresses that while turnover is systemic, case managers can mitigate impact through education and preparation, balancing “assertiveness with kindness, courtesy, and empathy.”
